Classroom dates: June 17, 18, 19, 20 Time: 3:00 PM - 5:00 PM Location : Oakridge 9th Grade Campus
Lab Dates: July 15, 16, 17,18, 19, 22, 23, 24, 25, 26, 29, 30 TIME: 1:00 PM - 5:00 PM

Students must attend the Classroom and Classroom/Lab Phases in order to receive a “Certificate of Completion”.
The Permit Test will be given at a DPS office of the parent's choice. Parents will be required to take the students to the DPS office to get their Permit at a cost of $16.00. Students will also need to be photographed and fingerprinted at the DPS office.We will provide students with applications for learner permit in class.
Upon successful completion of this class students will receive a “Certificate of Completion” (DE964E form) for driver’s license and insurance purposes.Students will also be mailed a certificate for the completion of the Drug and Alcohol Driving Awareness program.
Students will need the following items when testing at the DPS office of their choice:
- Verification of Attendance (VOE form). Students can pick this form up from their grade level offices at school. (Make sure to get last (2) weeks of school.)
- Birth Certificate – Certified (with state seal)
- Social Security Card – Original
- If not a US citizen, you must bring a valid Passport/Visa or Resident Alien Card
- Proof of Residency – 2 forms
- A copy of Birth Certificate and Social Security Card
- DL-40 Form/Application
- DL-14 Application
- DE964E Form (will be given to students first week of class)
