DSC Care Coordination, But I'm an Educator!



Think outside the box. For students to be successful everyone must recognize those students in need or complications associated with chronic conditions. Keeping students engaged and ready to learn, incorporates care coordination and student focused learning while addressing the mental health issues. The focus is on chronic conditions but will include some acute conditions/emergencies occurring in the classroom.
The objectives of this workshop is to:
Understand the role of the school nurse
Strategies to a coordinated system, teacher role
Planning and development of a coordinated system, everyone’s job
Addressing chronic conditions and emergencies (acute conditions)
o Diabetes
o Seizures
o Asthma
o Allergies/Anaphylaxis
o Oral Health Issues
Team-based approach to family and student and engagement
Student focused care
Academic achievement and absenteeism with chronic conditions
