NWAESC(2095)---Transition Planning for Students with Significant Disabilities***ZOOM***
This training will look at some assessments and tools that can provide information needed to write meaningful transition plans for students with significant disabilities. We will include information and resources on how to write post-secondary goals and share examples of the required goals within transition plans, as well as ideas for family engagement and interagency collaboration essential components of effective planning. Ideas for options for post-school outcomes will be shared, as well as examples of AR students who have reached positive post-school outcomes.
