SCSC VIRTUAL- Transition Planning for Students with Disabilities



This training will look at some assessments and tools that can provide information needed to write meaningful transition plans for students with significant disabilities. We will include information and
resources on how to write post-secondary goals and share examples of the required goals within
transition plans, as well as ideas for family engagement and interagency collaboration—essential
components of effective planning. Ideas for options for post-school outcomes will be shared, as well as
examples of AR students who have reached positive post-school outcomes.


Important Session Information:


For member districts of the SCSC Professional Development Consortium the fee has been waived. Districts that have not paid for nurses and/or paraprofessionals will pay the listed fee. In the event of 

cancellation, registrants will be notified by email (address tied to escWorks). Participants are not allowed to bring guests (such as children or spouses) to any trainings.
