Person-Centered Thinking and Transition Assessment





Learn how your district can implement the strategies to create a transition assessment based on a person-centered approach. The law requires that assessment be used to develop the IEP, yet districts struggle with getting accurate transition assessment for students who have difficulty communicating and/or setting achievable goals. . The person-centered assessment provides a description of the individual that includes what others like and admire about the person, things that are important to and for the person, characteristics a support person should have, and information on how the individual communicates with words and behaviors. (Information presented in this session was obtained during a yearlong pilot project done in collaboration with Texas districts and the Texas Institute for Person Centered Planning).  Session material costs are partially funded through state or federal grants.
