Transition Assessment: Using the Life Plan Approach





Learn how your district can implement the strategies to create a transition assessment based on a person-centered approach using the Life Plan method. IDEA requires that age-appropriate transition assessment is used to develop the postsecondary goals and transition plan as part of the IEP, yet districts struggle with getting accurate transition assessment for students who have difficulty communicating and/or setting achievable goals. This person-centered assessment provides a detailed approach for facilitating a transition-focused conversation which will help guide and assist with developing a sustainable adult schedule for students with disabilities.
