Using the Life Plan Transition Assessment





Participants will be provided with an introduction to person-centered planning (PCP), the PCP in the context of transition assessment, the functional vocational evaluation, and 18+. Training will be conducted on the Life Plan as a PCP, planning the test battery, the administration of the PCP, and data analysis. Participants will learn how to use the results to develop the transition postsecondary goals, the annual goals and objectives, the coordinated set of activities, and the role of the PCP in the Personal Graduation Plan and the Summary of Performance. Participants will be taught the report writing options, and participate in a simulation for planning, administration, and report writing.
